COVID-19 Symptom Review 

In the last 48 hours, have you had any of the following NEW symptoms?

  1. Fever of 100 F (37.8 C) or above, or possible fever symptoms like alternating chills and sweating
  2. Cough
  3. Trouble breathing, shortness of breath or severe wheezing
  4. Chills or repeated shaking with chills
  5. Muscle aches or Fatigue
  6. Congestion or runny nose
  7. Sore throat
  8. Loss of smell or taste, or a change in taste
  9. Nausea, vomiting or diarrhea
  10. Headache
  11. None of the above

Do you have any of the following possible emergency symptoms?

  1. Struggling to breathe or fighting for breath even while inactive or when resting
  2. Feeling about to collapse every time you stand or sit up (floppiness or lack of response in a child under age 2)
  3. Inability to stay awake or wake up
  4. None of the above

Call 911 or your local emergency number immediately if you develop symptoms like:

  1. Ongoing pain or pressure in your chest
  2. New Confusion
  3. A bluish color to your lips or face
  4. Struggling to breathe
  5. Feeling about to collapse every time you stand or sit up

If your health care provider suspects or confirms that you have COVID-19:

  1. Don't leave your home, except to get medical are, until cleared by your health care provider.
  2. Stay in a specific room away from others in your home, and use separate bathrooms, if possible.
  3. Do not share household items
  4. Wear a mask around others if contact is necessary
  5. Contact your health care provider for more advice if symptoms continue or get worse, or if new symptoms start
  6. Notify employee or occupational health at your work before returning to work

Our Fees

Our current fees, which are due in full at time of service are:

$90 for 1 week

$160 for 2 weeks

$310 for 4 weeks
